Community care plays an important role in building healthy communities. Community care means the mobilization and combination of formal and informal resources of the community. It aims to provide a supportive network for the persons and their families who need long-term care. The project focused on the Diabetes Mellitus patients and families of Fu-Sing neighborhood. It combined the community citizens of special skills. Through the empowerment process, it enhanced citizen participation, created a safe and supportive environment, and reformed the health care condition. The project involved three phases: the first phase concentrated on assessing the inner and outer resources of the community as well as analyzing its problems; the second phase worked on building a workshop and resource network; the third phase centered on evaluating the effect and defect of the whole project. The purpose of the project was to enlarge multisectoral services, to create a safe, energetic, and hospitable community, and to provide experiences for those who devote themselves to the work of building tealthy communities.
