The vessel tube is one of the important indicators of care quality in HD. Poor care leads to the waste of medical resources and affects the quality of care. We found that the total care rate for tube insertion in HD was 53.47% from Sep. to Oct. 2008. Our analysis revealed that there was a lack of operation guidelines: nursing instruction sheets were too complicated, containing contradictory information, and not available in wards. We created "The Blueprint of Patient Care for Tube Insertion in HD Patients". We saw that nursing instruction sheets were reorganized and revised. We built up the check-up system to improve total care for tube insertion in HD. The total care rate for tube insertion in HD was 92.44% from Nov. 2008 to Mar. 2009.We not only ameliorated the quality of nursing care and increased the caring ability of patients and health care-providers, but also upgraded the satisfaction rate of patients, health care-providers, and nurses.
