人口快速老化,高齡社會面臨的醫療照護挑戰,除失智、失能之後續長期照顧服務外,防止健康、亞健康長者走入不適當的健康行為,亦是重要的議題,而發展健康識能為必要的行動策略之一;老年人健康識能技能普遍偏低,且目前國內針對健康識能用在老年族群的概念文獻較為缺乏,亦無發展老人健康識能評估工具,故本文應用Walker及Avant概念分析的步驟進行健康識能概念分析,分析比較概念屬性的相同點與相異點,加以整合後,其定義性特徵為:(一)是一組具有層級區別的技能,用以獲取、理解、評價、應用健康訊息或知識;(二)此技能會因環境因素、個人因素、情境因素的影響而有所變動;(三)目的是用以增強健康行為動機與執行健康決策能力。但其場域為老年族群,處在身、心、社會功能退化的狀態,又因個人一般識讀能力的狀態不同,其健康識能的技能樣態更為複雜化,影響老年族群對健康知識的了解、慢性病照護與自我管理、健康照護與預防服務的可近性、健康消費支出多寡等健康行為與決策結果。期待經由健康識能概念的澄清,有助於未來建置老年人相關評估工具與發展有效的介入方案。
Rapid population aging has resulted in challenges for the medical care of older adults, including follow-up long-term care services for dementia and incapacitation and prevention of inappropriate health behaviors among healthy and subhealthy older adults. Health development is a critical topic, and health literacy is a necessary strategy. The health literacy of older adults is generally low, and related conceptual studies and assessment tools are lacking. Therefore, this study applied the concept analysis process provided by Walker and Avant to verify definitions; identify defining attributes, antecedents, and consequences; and provide examples of model, borderline, and contrary cases. This concept analysis provides clarification and facilitates the establishment of related assessment tools and effective intervention programs for older adults.
