當醫療院所進入全面競爭的時代,醫療品質是醫療產業競爭的重要策略指標。對復健中心經營而言,必須充分瞭解影響復健治療工作同仁工作滿意因素,以便提高復健品質。本研究應用橫斷式問卷調查法。問卷乃參考明尼蘇達短式滿意度問卷(MSQ)、組織承諾(OCQ)與護理工作指引修正版(NWIR)等問卷,修正發展成適用復健中心情境問卷。再針對台灣醫療院所與機構的復健治療單位進行調查,並應用結構方程(SEM)分析。分析結果顯示:復健治療單位的工作環境對復健治療同仁的工作滿意有顯著影響,工作滿意進而影響復健品質。但工作環境對組織承諾無顯著影響,組織承諾卻受到工作滿意顯著影響;同時,組織承諾對復健品質亦無顯著影響。在工作環境構面中,以與醫師關係負荷因素最高。建議醫院管理應致力於改善工作環境,特別是與醫師關係,有助於提昇醫療品質及改善復健經營。
During the era of competition, care quality of medical institution is an important indicator of medical industry competition. To improve the quality of rehabilitation, management of rehabilitation center must meet the impact factors of satisfaction of rehabilitation therapist job. This study uses cross-section questionnaire survey. Questionnaire is determined by reference to the Minnesota Satisfaction Questionnaire Short Form (MSQ), Organizational Commitment (OCQ)and Revised Nursing Work Index (NWIR) and other questionnaires, modified to be suitable for rehabilitation center. By using the questionnaire, we investigate the rehabilitation centers of Taiwan and do analysis by Structural Equation Modeling (SEM). The result shows working environment is significant to therapist’s job satisfaction, and job satisfaction is significant to the rehabilitation quality. But working environment is not significant to the organizational commitment. The job satisfaction is significant to organizational commitment. On the facet of working environment, the relational load factor of relation with physicians is the highest. This study suggests that the medical institution should improve working environment, especially the physician - therapist relationship, will enhance the quality of rehabilitation and management.
