     目的:注意力不足過動症是最常見的一種兒童精神疾病,目前在臺灣對注意力不足過動症兒童之表現、成因的了解有限,故本研究的目的為獲得7到12歲學生的注意力不足過動症之盛行率、次分類的問題並探討DSM-IV注意力不足過動症次分類的相關因子。 方法:針對臺北市某區域國小年齡範圍為7~12歲之全校兒童進行研究,第一階段以兒童活動量表(父母版與老師版)作篩選,以大於85%,作為初步盛行率之標準。疑似個案中之24.6%,進一步接受Mini Kid (Mini International Neuro-psychiatric interview for children and Adolescent) 診斷會談,以確定是否為注意力不足過動症,次分類個案之生物遺傳疾病狀況、注意力不足過動症之外向性行為 (externalizing behavior)、內向性行為 (internalizing behavior) 分佈是主要的分析內容。 結果:根據老師版兒童活動量表,篩選出疑為注意力不足過動症之盛行率為8.4%。第二階段由醫師診斷出,注意力不足過動症之三個次分類比例:即混合型:不專心型:過動衝動型是29.5:14.5:1。確定診斷的次分類中,混合型的比不專心型注意力不足過動症,有較多的遺傳家族病史、問題行為與暴力傾向。 結論:某臺北市國小的注意不足過動症之兒童人數多,混合型的人數最多,在精神病家族史、問題行為上,都比不專心型的注意力不足過動症來得嚴重。
     Objectives: ADHD is the most common disorder in child psychiatric patients. Data on the characteristics of Taiwanese children with ADHD are lacking. This study examined the prevalence of ADHD among 7 to 12 year-old students in an elementary school in Taipei and also investigated the differences in psychiatric history and problematic behavior in the inattentive and combined subtypes in the students with ADHD. Methods: Students at an elementary school in Taipei were screened by using an activity level checklist (teacher and parent version). The cut off percentage was 85%. Prevalence data was based on the results of screening. One psychiatrist confirmed suspected cases of ADHD using the Mini Kid (Mini International Neuro-psychiatric Interview for Children and Adolescents) interview. Confirmed ADHD cases were divided into two subtypes (combined and inattentive type) to assess the implications of parental family psychiatric history, maternal delivery complications, externalizing behavior and internalizing behavior, school performance and interpersonal relationships between the two groups. Results: The prevalence of ADHD was 8.4%. The percentage of cases in the 3 subtype ratio (ADHD-C: ADHD-I: ADHD-H) was 29.5:14.5:1. Subjects with the combined type had a higher frequency of family genetic psychiatric history, more externalizing problematic behavior and more aggressive tendency than the inattentive type. Conclusion: This study found a high prevalence of ADHD among students at this elementary school in Taipei. Students with combined type ADHD had a higher rate of family psychiatric history and more problematic behavior than those with inattentive type ADHD.
