題名:以依賴音量大小的聽覺誘發電位(LDAEP)N1/P2組合作為SSRI類藥物治療憂鬱症的預測指標

     目的:血清素(serotonin) 的功能異常被假說為憂鬱症的病理角色之一,但仍有相當數目的憂鬱症個案,以SSRI治療無理想反應者。因此,中樞神經系統血清素活動力的指標物可能可以作為預測憂鬱症藥物治療效果的指標。有越來越多的證據顯示較強的依賴音量大小的聽覺誘發電位(loudness dependency of auditory evoked potentials, LDAEP) 是中樞神經系統血清素活動力低下的指標。因此本研究的目的在於研究LDAEP 與SSRI 治療憂鬱症的關係。 方法:收集八十位未服藥或停藥至少四週的憂鬱症個案在使用SSRI藥物治療前施以LDAEP的檢測。憂鬱症狀的嚴重程度,分別在治療前與治療四星期後以漢米爾頓憂鬱量表評分。 結果:治療前具有較強LDAEP 的病患比較弱LDAEP 的病患在四星期的治療後,憂鬱症狀有較顯著的減輕。 結論:我們的研究證實具有較強LDAEP的憂鬱症患者對SSRI治療有較好的反應,LDAEP是一個可預測憂鬱症對SSRI治療反應的工具。
     Objective: Although serotonergic dysfimction is thought to play a pathogenic role in depression, but there are a considerable number of non-responders to SSRI treatment of depression. There is a growing amount of evidence that a strong loudness dependency of auditory evoked potentials (LDAEP) is an indicator of low central serotonergic activity. The goal of this study was to analyze the relationship between LDAEP and SSRI treatment response in depressive patients. Method: Eighty patients with depression who were drug-naive or drug-free more than 4 weeks were enrolled and LDAEP were tested prior to SSRI treatment. Depressive symptoms were scored according to the Hamilton Scale for Depression (HAM-D) at the beginning and four weeks after the initiation of treatment. Result: Patients with a strong pre-treatment LDAEP had a significantly greater decrease of depressive symptoms (HAMD) after 4 weeks than patients with weak LDAER Conclusion: This study has confirmed the hypothesis that strong LDAEP are related to a favorable response to SSRI treatment in depression. These findings suggest that the measurement of LDAEP is a promising tool for the prediction of response to SSRIs in depression.
