This case reports a Type 2 Diabetes Mellitus (DM) patient who settled in Intensive Care Unit ( ICU) expecting for a third pancreas transplantation. The patient was fearful of another failed surgery. Using Roy's adaptation model to evaluate the patient, it was found that he had pain, infection, anxiety, lack of knowledge, and disturbed sleep pattern. Patient was encouraged to describe his anxiety. Nurses used skills like listening, reinforcing patient's faith, and providing professional knowledge in releasing patient's anxiety. By knowing patient's physical pain, anxiety and needs during nursing process, better individual nursing care can be provided and help patients go through illness and anxiety.
