題名:穿戴不同足踝護具在踝關節受力及穩定度之差異

踝關節在扭傷後,醫療人員會建議患者使用足踝護具進行踝關節的保護,然而市面上足踝護具的選擇種類繁多且價格差異極大。從過去文獻發現足踝護具對於踝關節是有一定程度的保護效果,但都侷限在動作表現、關節角度、地面反作用力及肌肉反應上面的比較,對於踝關節內部受力的影響並無文獻進行探討,因此本研究目的將透過數學模型計算年輕業餘運動員在裸足、穿戴軟式足踝護具與綁帶式足踝護具,在踝關節內部受力和穩定時間的差異。本研究利用動作分析儀器與力板收取單腳著地資料後,再藉由數學模型計算出踝關節受力。本研究結果在踝關節受力與地面反作用力表現上,穿戴綁帶式足踝護具後垂直向地面反作用力與垂直向踝關節受力都會上升,內外向踝關節受力會下降與裸足有明顯的差異。而軟式足踝護具與綁帶式足踝護具相同的地方在於內外向的穩定時間與裸足相比都有顯著下降的結果,在地面反作用力與踝關節受力表現上雖然都有與綁帶式足踝護具有相似的趨勢,但和裸足間都沒有統計上的顯著差異。本研究結論:由本研究結果得知穿戴兩種足踝護具後皆會增加垂直方向的踝關節受力,以強化踝關節穩定度。而穿戴綁帶式足踝護具在內外側方向可以顯著減少踝關節受力及加快穩定踝關節,以對踝關節達到保護的效果。
Physical therapists and doctors usually suggest patients with ankle sprain to use the ankle brace to protect their ankle joints. However, there are many types of ankle braces to choose, and the price range is quite large. Previous studies found that ankle brace may protect the ankle joint in terms of performance, range of motion, ground reaction force and muscle reaction. Yet, no study demonstrated the ankle joint force after wearing the ankle brace, especially for basic and advanced ankle braces. Therefore, our purpose of this study was to compare the differences between two kinds of ankle braces (soft ankle brace and lace-up ankle brace) in terms of stabilization and joint force. In this study, the motion analysis system and force plate were implemented to collect the data. Mathematical model was conducted to calculate the ankle joint force. The results showed that wearing lace-up ankle brace could increase the vertical ground reaction force and vertical ankle joint force, while decreased the medial-lateral (ML) ankle joint force. Compared to barefoot, wearing soft ankle brace or lace-up ankle brace were able to reduce the stabilization time in ML direction respectively. Although wearing soft ankle brace had the similar tendency as wearing lace-up ankle brace in stabilization time and ground reaction force, no statistical significant difference was noticed compared to barefoot. This study concluded that wearing either soft ankle brace or lace-up ankle brace would increase the ankle joint force in vertical direction to stabilize the ankle joint. Additionally, in ML direction, wearing lace-up ankle brace could significantly reduce the ankle joint force and quickly stabilize the ankle joint to get an effect of protecting ankle joint.
