目的：本研究旨在探討長期照顧機構採用遠距照護之現況與障礙因素。方法：以全國長期照顧機構為對象，採地區別分層比例抽樣500 家發放問卷。共計回收272 份問卷，有效問卷244 份，有效回收率48.8%。結果：發現多數（82%）長照機構未採用遠距照護服務；逾七成暫無採用計劃。機構式遠距照護採用的重要障礙依照得分高低依次為政府法規、成本因素與技術因素。影響機構式遠距照護採用意願的障礙因素則依次為組織因素、員工因素、政府與法規。結論：政府應規劃明確的法律條文規定和補助辦法，建立機構和系統業者的媒合平台，並獎勵標竿機構。機構則可強化教育訓練及組織改變措施，讓員工了解系統的正面效益，減少和既有照護模式磨合的時間成本。

Objectives: This study determined the status quo and barriers to telecare adoption in long-term care institutions. Methods: This study used a survey approach. The study population was long-term care institutions in Taiwan. Stratified proportional sampling based on regions was conducted to select samples. A total of 500 institutions were investigated and 272 questionnaires were returned, including 244 effective responses, yielding a 48.8% of effective response rate. Results: The results showed that most long-term care institutions (82%) did not adopt telecare. Greater than 70% of long-care institutions currently have no plans for adoption. Major barriers to adoptions were government regulations, cost, and technology factors in the order of barrier scores. The major barrier influencing the intentions of adoptions were organizational factors, followed by staff factors, as well as government and regulations. Conclusions: The current study suggests that the government arrange clearer legal provisions and financial assistance, establish a matching-service platform, and benchmark institutions to increase the intentions of telecare adoptions among long-term care institutions. Moreover, long-term care institutions can enhance education and training programs so that staff can understand the positive benefits of this system to reduce the run time costs with the current processes.
