題名:運動對機構住民死亡前臥床天數影響之相關研究

臺灣的老年失能人口急速上升,但臺灣民眾對於失能者仍需維持運動習慣之觀念普遍缺乏,因此造成老人失能後不僅身體活動受到影響,甚至自我照顧能力下降,最終長期臥床至死亡。因此如何在生命的末期,減少受到病痛的折磨,保有較佳的自我照顧能力,縮短死亡前的臥床天數,減少逝世前臥床病痛折磨,為本研究感興趣之議題。本研究採回溯縱貫性次級資料觀察性研究,以臺中市聘用專任健身教練之護理之家住民次級資料作為研究素材,納入2012年3月1日至2020年6月30日入住至少6個月之已死亡住民為研究對象,共計154人,排除從入住到死亡皆為嚴重失能PPS得分40%以下之長期臥床住民、確診癌症末期住民後,實際收案71人。觀察護理之家住民個人因素、營養狀況及身體功能與參加運動頻率,進而瞭解住民參與運動對死亡前住院天數的影響。研究結果發現1.罹患兩種以上疾病者其死亡前臥床天數較一種疾病者短。2.運動可以縮短臥床時間,但運動頻率高與低無明顯差別。本研究結果期待能讓民眾了解運動對老年生活品質的重要性,養成規律且適度運動習慣、保持健康,本結果亦提供長期照顧機構經營者、管理者作為經營的參考,透過機構人員帶領長輩在入住機構後,仍保有運動習慣,在生命的末期失能程度低,死亡前的臥床時間短。
The elderly disabled population in Taiwan is rising rapidly, but Taiwanese generally lack the concept that disabled people still need to maintain the habits of doing exercises. This deficiency of concept not only causes influences on physical activities, but also declines the elders' self-care ability, and eventually made the elders lie in bed to death after becoming disabled. As explained above, to overcome this situation, this study is going to discuss how to alleviate the torture of illness, let the patients be able to keep a better self-care ability and shorten the days in bed before death at the end of life. Research design: We did longitudinal research, using the secondary data of nursing home residents who employ full-time fitness coaches in Taichung City as materials. Took those who had lived in the nursing home for at least six months and passed away during 1st March 2012 to 30st Jun 2020 as our objects of study, 154 people in total. Excluded those who were diagnosed terminal cancer and those who were rated below 40% on Palliative Performance Scale (PPS) from living in the nursing home to death, our objects of study were 71 people. By observing the nursing home residents' personal reasons, nutritional status, body functions and their frequency of participating in sports, we get to understand the effect of residents' participation in sports on the number of days living in hospital before death. Research result: 1) Those who suffer two or more diseases had fewer days in bed before death than those with one disease. 2) Doing exercises can cut down bedridden days before death, but the frequency of doing exercises doesn't influence anything. The result of this study expects to make the public understand the importance of doing exercise to the quality of life of the elderly, develop a habit of regular and moderate exercise and stay healthy. The result also provides the operators and managers of long-term care facilities a reference in business. With the leading of facilities' staffs, elders may keep a habit of doing exercises, which could low the level of disability at the end of life, shorten the time in bed before death after living in the facilities.
