Because the increase of elderly population and chronic diseases, long-term care plays a very important role in the medical system. Due to its complicated and numerous service types, long-term delivery service can't offer a comprehensive services under the current health insurance payment system. The occupancy rate of acute beds for chronic patients in hospital is quite high, this results in negative impact on most of hospitals. Results showed that: long-term care facilities in hospitals were almost affiliated with medical centers, public hospitals. 61% hospitals got impacts under the NHIP. Negative impacts included delay of length of stay (LOS) for chronic patients, need of more human resources for oxygenator users and cutting of reimbursement for chronic beds. These impacts also urged hospital manager to establish home care and day care center for releasing high occupancy rate of beds by chronic patients. We recommend that the hospital manager should improve audit of pre-admission and comprehensive plan of discharging patients. It is also important to establish a better post-discharge system for chronic patients, such as nursing home and transfer system for keeping a better relationship between physicians and patients.
