目的：報告一例難治型強迫症患的治療過程並回顧文獻，討論難治型強迫症患者 的藥物治療對策。病例報告：一位２１歲女性強迫症患者，二年前開始出現強迫症狀，最近 十個月以來症狀加劇，曾至多家醫院求診，對於fluoxetine， fluvoxamine或 fluvoxamine 併用sulpiride療效不佳，後來服用fluvoxamine併用低劑量risperidone約一個月後，強迫 症狀大幅改善。結論：治療難治型強迫症患者，可考慮併用低劑量risperidone，增強血清 素再吸收抑制劑（serotonin reuptake inhibitors, SRIs）的治療效果。　

　　　　　To report a case of refractory obsessive-compulsive disorder and discuss risperidone augmentation of fluvoxamine therapy in such case. Case Report: A 21 year-old woman with a diagnosis of obsessive-compulsive disorder had a two-year history of obsessive-compulsive symptoms. Her symptoms had become exacerbated in the ten months prior to admission, and she had sought help at several hospitals for help. Fluoxetine, fluxoxamine, or fluvoxamine combined with sulpiride had been prescribed but in vain. Low dosage of risperidone augmentation of fluvoxamine resulted in significant improvement of her symptoms one month later. Conclusion: Low dosage of risperidone augmenation reduces obsessive-compulsive symptoms in patients with refractory obsessive-compulsive disorder. (Full Text in Chinese)
