本文描述一位十歲小男孩爬樹玩耍時不慎跌落，樹枝從肛門插入導致腸破裂，需藉由暫時性的腸造口，幫助身體復原之護理過程。筆者以羅氏適應模式為評估指引，透過觀察、會談、身體評估、查閱病歷等方式收集資料，發現個案有傷口疼痛、感染、身體心像紊亂等健康問題。筆者於2009年3月21日至4月6日個案住院期間，針對以上的健康問題擬定護理計畫，進行傷口疼痛、感染及身體心像紊亂等問題的照護措施。此外，筆者協助個案及家屬面對造口問題，讓此意外傷害兒童能由衝突期順利轉換為重建期，使個案能重新適應原有的生活型態，完成其發展階段的任務，及執行應有的社會化功能。

This paper is to describe the nursing process in which a temporary colostomy was given to a ten years old boy to repair his intestine. The boy fell from a tree and injured his intestine. The author used Roy adaptation model as the assessment guide to collect data by observation, conversation, physical assessment, and checking medical history to identify wound pain, infection, and body image disturbance as the case health problems. The author designed nursing plans and executed interventions for wound pain, infection, and body image disturbance during hospitalization from March 21 to April 6, 2009. Additionally, the author assisted the case and helped the family face the colostomy problem. The author also helped the child transfer from a period of conflict into reconstruction and readapted the original lifestyle to accomplish the development stage in social function behavior pertaining to his age.
