題名:自閉症兒童意圖能力之研究

本研究提出自閉症患者早期發展中行動意圖能力缺陷(action-based intentional ability deficit)的假說,選取生理年齡在三歲半以下的自閉症兒童為主要對象,並選取生理年齡、發展年齡相當的發展遲緩兒童為配對組,生理年齡在13-15個月,以及18-20個月的正常發展嬰幼兒為控制組,從意圖的觀點切入設計三項測量意圖能力的實驗。研究主要的假設有二:
1. 自閉症兒童在相互注意協調能力、意圖溝通能力和動作意圖能力上,皆落後於相配對的發展遲緩兒童與控制組嬰幼兒,而後兩組兒童,若已有相當的發展年齡,在這三項能力上,沒有明顯差異。
2. 若進行個別比較,已發展相互注意協調能力的自閉症兒童,他們在發展意圖溝通能力與動作意圖能力的比例仍有限且低於配對組及控制組,而後兩組兒童,若有相當的發展年齡,發展出意圖溝通能力與動作意圖能力的比例相當。同時,三項意圖能力的發展順序,自閉症組兒童亦與其他三組之間有所不同,後者在三項意圖能力的發展順序應有一致的趨勢。
對第一項假設而言,結果發現:在相互注意協調能力方面,自閉症組兒童在低層次的相互注意協調能力不如其他三組,在高層次的相互注意協調能力亦落後於發展遲緩組兒童與大正常組嬰幼兒。在意圖溝通能力方面,自閉症組兒童在層次一的意圖溝通能力未落後於其他三組,但在層次二意圖溝通能力的表現則明顯有落後的現象,即使少部份能達到層次二意圖溝通能力的自閉症組兒童(有7名),大部份(有6名)在使用不同溝通方法的能力仍極有限。在動作意圖能力方面,自閉症組兒童的表現,並未掌握瞭解他人「未完成」意圖的型態,而發展年齡相當的大正常組與發展遲緩組兒童,則表現出瞭解的反應趨勢。
對第二項假設而言,結果發現:發展出相互注意協調能力的自閉症組兒童中,沒有任何一位同時發展出後兩項意圖能力,自閉症組兒童出現意圖溝通能力或動作意圖能力的比例低於其他三組。此外,正常嬰幼兒與發展遲緩兒童發展出三項意圖能力的順序有明顯的一致性,但自閉症組兒童在意圖能力的發展路徑,卻並不一致。
綜合來說,由意圖的觀點切入,自閉症兒童早期發展過程中行動意圖缺陷的假說,由本研究的結果看來,已得到初步的支持。這項論點,擴充過去學界對自閉症患者早期的發展障礙,多從相互注意協調能力缺陷觀點的侷限性,也更進一步從發展歷程的觀點,指出他們在發展過程中可能的不同路徑。
最後,筆者提出後續研究的規畫,以更進一步深化本研究所提出的論點。在臨床的應用上,亦提出在早期診斷與早期療育中的可能方向。
