本文描述一位70歲老人罹患腦中風面臨吞嚥障礙及肢體乏力引發相關健康問題之護理經驗。護理期間自2008年1月26日至3月10日，筆者運用Gordon十一項功能性評估確立主要的健康問題有自我照顧能力缺失：沐浴、如廁，身體活動功能障礙、吞嚥障礙及身體心像紊亂等健康問題。先讓個案學習自我照顧與訓練肢體力量，再透過吞嚥訓練成功移除鼻胃管，恢復由口進食的能力，拾回自信心，而更願意積極參與復健，學習自我照顧，出院前由完全臥床恢復到使用拐杖行走、到浴室清潔身體與如廁。期望此護理經驗有助提升護理人員對腦中風病人之照護。

This article aimed at discussing the experience in caring for a patient with swallowing dysfunction and limb weakness induced by cerebral vascular accident (CVA).The duration of caring for this patient was from January 26 to March 10, 2008. The author used Gordon's 11 functional items to collect data and assess this patient. The major health problems of this patient were bathing, toileting, swallowing dysfunction, body image disturbance, physical disability etc. Firstly, exercises for strengthening affected limbs and self-care Training were performed for this patient. Secondly, swallowing training was implemented to assist the patient in feeding himself safely. Finally, the nasogastric tube could be removed. By establishing confidence and positive body image, our team assisted this patient build self-confidence and receive rehabilitation programs activity. Before being discharged from the hospital, this patient could walk with a walking stick as well as going to the bathroom for showering and toileting. By sharing this care experience, we hope to improve nursing care quality for cerebral vascular accident patients.
