題名:以系統性文獻回顧檢視瑜珈介入在癌症護理實務的應用

背景 : 瑜珈介入近年來被廣為運用於癌症之身心治療及緩解病患之症狀困擾,但目前國內尚無完整之文獻評價其臨床實務成效。 目的 : 本文旨在搜尋以瑜珈為介入措施於癌症照護之臨床實驗性研究,檢視其適用癌症種類、操作手法、身心照護成效及研究方法品質,以提出瑜珈介入方案於癌症護理實務之運用。 方法 : 以系統性文獻回顧法搜尋 2000至 2010十年間發表於 MEDLINE、PubMed、Cochrane Library、 Cancerlist、ProQuest及 CEPS等資料庫,使用關鍵字瑜珈( Yoga)及癌症( Cancer或 Neoplasm),共 96篇,選取符合篩選標準並排除重複出現,共找出 11篇瑜珈介入之隨機分派或具控制對照組之臨床實驗性研究( RCT及 CCT),進行文章的分析與整理。 結果 : 11篇研究中發現,瑜珈介入可顯著改善癌症病患之焦慮、憂鬱、疲憊、睡眠及促進生活品質等正向效益。瑜珈治療可能因瑜珈的類型、研究工具、樣本數和介入的時間等,影響研究的結果,故無法進行統合性分析,成為本篇文獻回顧之限制。 結論 : 本文結果可提供瑜珈介入適用於癌症的症狀困擾及操作手法訊息,有助於臨床護理人員學習瑜珈介入照護措施,並發展瑜珈介入標準作業流程。檢視研究方法品質,提供瑜珈介入未來研究方向及研究設計建議。
Background: Yoga intervention is increasingly being employed in healthcare to promote mind-body health and relieve symptom distress in cancer patients. Little literature related to its effectiveness in clinical practice has been published in Taiwan. Purpose: The authors reviewed published clinical experimental research on yoga as a cancer patient care intervention. The authors considered cancer patient type, operating practices, mind-body health benefits and research method quality in order to propose a formal yoga intervention program for application in cancer nursing care. Methods: This study used a systematic review method and searched six databases including MEDLINE, PubMed, Cochrane Library, Cancerlist, ProQuest and CEPS. The search focused only on articles published between 2000 and 2010 using the keyword “Yoga” together with either “Cancer” or “Neoplasm”. An initial 96 articles were initially identified. After filtering based on inclusion criteria and excluding duplicates, the authors selected the remaining 11 articles that contained either a randomized clinical trial (RCT) or controlled clinical trial (CCT) for further analysis and synthesis. Results: All 11 studies supported the ability of yoga to ameliorate anxiety, depression and fatigue significantly and enhance quality of sleep and daily life in cancer patients. A limitation of this study is the difficulty of doing a meta analysis due to the inherent diversity among reviewed studies in aspects such as yoga intervention type, research instruments used, sample size targeted and intervention timing. Conclusions: This study recommends using yoga intervention to relieve cancer-related symptoms and provides operational information to help nurses learn yoga as an intervention for clinical practice and develop yoga into a standard procedure. Reviewing the quality of research methods among studies offers a consideration in the future research design of yoga studies.
