本文探討一位機械性損傷的28歲青年，在歷經多次清創手術後仍面臨右手肘下截肢手術之照護經驗。筆者於2009年4月9日至5月17日，藉由直接照顧、會談及傾聽等建立良好的護病關係，並以羅氏適應模式進行資料收集，確認個案主要有自我照顧能力缺失、疼痛、身體心像紊亂、預期性哀傷等四項護理問題。經由全程照護並運用相關資源，協助個案與家屬自術後至恢復期皆能獲得支持，個案最後終能建立其正向身體心像及調適。

This article is to discuss the nursing experience of a young male undergoing debridement and amputation surgeries due to mechanical injury. After assessing the patient with Roy's Adaptation Model, 4 nursing problems including self-care deficit, acute pain, body image disturbance and anticipatory grief were identified. Through providing nursing care and related resources to the patient and his family, the authors finally helped the patient accept the disease and establish positive body image.
