目的：人口高齡化增加了長期照顧的需求，有多數的個案必須整合家屬共同照顧，需要研發支援群組協同合作的健康管理服務系統。方法：以系統實作的方式建立標準化個人健康紀錄，收集個案健康管理所需的資料，以帳號授權的方式提供家屬共同紀錄和查詢，輔助家屬進行討論與決策，並且加強資訊安全和隱私保護。系統模組主要包含家庭群組維護、健康資訊上傳下載、健康風險評估、健康風險維護和群組討論等五個子系統。結果：建立一套家屬健康管理服務系統，可以由個案或家屬匯入健保署健康存摺或是健康檢查、日常生理量測或服藥紀錄作為個人健康紀錄資料，本系統亦可協助推斷特定疾病的危險族群，家屬也可以透過平臺，查詢疾病衛教相關資訊，以利討論照顧措施。結論：長期照顧的資訊管理除了個案與醫護人員之外，還需要家屬共同投入協同合作，開發適當的群組軟體，可以協助家屬之間的資料收集與互動討論並支援決策，有助於提升照顧服務品質。

Objectives: Aged demographic structures increase demands for long-term care. Generally, family members provide care for one another. Therefore, a family health care management system incorporating group collaboration support is needed. Methods: A standardized, interactive personal health record system was implemented for collecting information necessary for health care management. The system provides family members with a method for recording and querying health-related information for discussion and decision-making, with enhanced information security and privacy protection through account management. The system includes the following modules: Family Group Maintenance, Health Information Upload and Download, Health Risk Assessment, Health Risk Maintenance, and Group Discussion. Results: Family members can import to the system their medical records from My Health Bank (provided by the National Health Insurance Administration), which includes health examination and daily physiological measurement records. The system can also assist in inferring risk-specific groups. Moreover, family members can search health education and disease-related information to facilitate discussion of care planning through this platform. Conclusions: In addition to patients and medical staff, long-term care requires support and cooperation between family members. The development of appropriate groupware may assist family members in collecting and exchanging information, thereby improving the quality of health care services.
