　　　　　目的：探討精神分裂症患者主要照顧者之負擔的盛行率與相關因素。這些實證資 料對提供更佳之精神醫療服務將相當有關。方法：有177位精神分裂症患者的主要照顧者接 受以結構式問卷進行之面談，以獲取其負擔與社會性支持之資料。負擔分為擔心感受與認 定之後果兩層面。各項負擔與支持題項之盛行率將予以分析，其負擔之相關因素將以邏輯 迴歸分析之。結果：只有一半（50%）左右之研究樣本認為獲得良好之支持。“疾病治不好 ”是7項負擔題項中有最高盛行率者（擔心感受：74.6%；認定之後果：65.1%）。另外有4 項，包括“經濟損失”，“生活常規受干擾”；“暴力行為之干擾”；“干擾家庭關係” ，也有高之盛行率，其範圍是35.6%至49.1%。在“擔心之感受”與“認定之後果”兩層面 ，各項負擔之盛行率相似。預測“擔心感受”之因素包括了“非第一次住院”，“病程較 長（>2年）”與“照顧患者之支持差者”等三項；預測“認定之後果”之因素包括了“病 程較長（>2年）”與“對照顧患者與自我感受之支持性差者”。結論：精神分裂病患者的 主要照顧者之各種負擔均有高之盛行率。病程較長與所獲得之社會支持差者，預測主要照 顧者有較明顯之負擔。對發展較佳之精神醫療服務方案，這些資料可供具體之參考價值。
　　　　　Objective: To investigate the prevalenc rates and related factors of burdens on chief caregivers of patients dignosed with schizophrenia. The empirical data are important for planning better psychiatric service programs. Methods: One hundred seventy-seven chief caregivers of consecutively admitted schizophrenic probands were interviewed using a structured interview schedule to obtain information about support and burdens. Burden was divided into two aspects of worry (W) and perceived effects (PE). The prevalence of burden and support items was explored. Factors related to various burdens were examined using logistic regression analysis. Results: Good support was reported for approximateely 50% of the studey participants."Disease incurability" had the highes tprevalence (W:74.6%; PE: 65.1%) among seven items considered burdens. The other three items were "bothered by violent behavior", "disturbed daily routine", and "economic loss", which also had high prevalence (range, 35.6% to 49.1%). The prevalence was similar in both aspects of W and PE for each item. Clinical variables of "non-first admission" and "longer duration of illness ( over 2 years)" as well as "poor support for caring patient" were related to greater amounts of W.C linical variable of "longer duration of illness (over 2 years)" and "poor support for sharing feelings, and for caring patients" were related to the PE. Conclusion: High prevalence rates of burdens were found in the chief caregivers of the patients with schizophrenia. These burdens were related to longer clinical courses and poor social support. This important information should be used to provid better psychiatric service programs.
