This report explores that the nursing care of a patient with C3-4 herniated intervertebral disc (HIVD) combined with spinal cord compression after a C3-4 laminectomy and fusion operation. During the nursing period from Mar. 2 to 12 in 2008, the data were collected as the patient’s conditions through physical, mental, social and spiritual levels by the observation, interview and physical assessment. The four major health problems of the client were ineffective tissue perfusion (peripheral); ineffective airway clearance; impaired physical mobility; and anxiety. The author gathered client information with the use of listening, company, empathy and continuing caring skills to realize the holistic nursing concept for establishing a good nurse-patient relationship. These interventions were made to restore respiratory function and to avoid muscle spasm by providing individual care and holistic disease information to assist the client to face spinal shock stage and adapt to physical-mental disturbance from disease. The client was transferred to a general ward successfully in Mar. 12, 2008. It is expected that this report related to the individual care experience can help staff nurses to understand and learn.
