癌因性疲憊為癌症或癌症治療中常見的副作用之一,具較高的盛行率且會影響患者整體的生活品質。癌因性疲憊是可能被治療的,藉由篩選分析可能因素後,給予適當的藥物進行治療即可緩解疲憊情形,目前使用的藥物包括有methylphenidate、modafinil、corticosteroids、抗憂鬱類藥物、Erythropoietin-stimulating agents及L-carnitine等藥物,臨床研究認為多數藥物對於疲憊具有緩解效果,但也有部分文獻認為其療效仍有待更多研究確認,且在使用劑量、療程及實際作用機制方面也需有更大型且設計嚴謹的臨床試驗結果證實。除西醫療法外,中草藥也為輔助與替代性療法的熱門選擇,許多的研究結果也證實人參、黃耆等補氣類藥材具有緩解疲憊等癌症治療副作用的效果,台灣亦有由黃耆萃取物研發而成的植物新藥-PG2,其以癌因性疲憊做為主適應症經TFDA核准為處方藥。目前用於治療的藥物除PG2外皆非以癌因性疲憊做為主要適應症,故期望未來有更多研究投入於疲憊治療藥物的研發。
Fatigue is the most common symptom related to cancer and its treatment. In addition, it is highly prevalent and may decrease patients' quality of life. Cancer-related fatigue(CRF) is treatable, and clinicians should screen and find the factors that cause fatigue and use appropriate medicine to treat the symptom. There are some studies about pharmacotherapy of cancer-related fatigue indicate that the drugs including psychostimulants, antidepressants, corticosteroids, Erythropoietin-stimulating agents and L-carnitine can reduce CRF effectively. On the other hand, some studies claim that based on statistics, there isn't a significant effect; more trials are needed to clarify the effectiveness. Larger and well-designed clinical trials are necessary to find the relevant dosage and treatment duration for fatigue patients. Other than western medicine, traditional Chinese medicine is a popular complementary and alternative medicine. Several studies have shown that Ginseng and Astragalus can reduce the side effects related to cancer treatments. A scientific research from Taiwan about Astragalus' extraction as a botanical drug, PG2, with its major indication to CRF, has been approved by TFDA. By designing and implementing various trials, it is possible that more pharmacotherapy drugs with major indication to CRF would be discovered and would benefit the society in the future.
