高齡化對整體社會照顧體系形成嚴重之衝擊,但亦對應產生許多產業機會與商業模式,然而,過去相關研究少有針對長照機構之商業模式議題進行深入探討。本研究目的為基於顧客價值觀點,進而探討長照機構之商業模式創新,採個案研究法並以商業模式之「顧客價值主張-關鍵資源-關鍵流程-利潤公式」為理論架構,其中顧客價值主張包含功能性價值、快樂性價值、情境性價值、關係性價值、以及倫理性價值等五構面,並挑選全球具代表性之長照機構為本研究之個案,包括歐洲之Orpea、日本之Sompo Care與Nichiigakkan、以及台灣之雙連安養中心,蒐集上述個案自2000年至2021年之長照服務與商業模式策略事件資料,並輔以專家深度。研究發現如下:首先,長照機構皆能注重與平衡發展顧客之五大價值主張,初期較重視照護功能性價值與友善環境營造之情境性價值,中期朝向倫理性與快樂性價值發展,近期注重住民與家屬之關係性價值。第二、在關鍵資源中,初期大都重視長照機構相關基礎建設之投入,以及跨領域專業之人力資源整合,中期朝向與外部資源進行整合,近期則注重投入新興科技之資源。第三、在關鍵流程中,初期強調機構環境建置與維護流程、住民照護流程、以及員工培訓流程等,中期重視提升照護品質流程,近期則轉以強調住民關係性價值流程、新科技照護流程以及與外部企業合作營運流程。最後,在利潤公式中,初期強調持續改善照護服務與環境,中期重視照護方式並營造快樂的環境,近期則轉以強調住民健康、安全、心理層面的居住環境、並更重視“人”與“科技”共存的新型養老方式。
Aging has a serious impact on the overall social care system, but it also brings many industrial opportunities and business models. However, there were few studies deeply exploring the issues of business model. The study aims to explore the business model innovation of long-term care institutes based on the perspective of customer values. The study adopts the method of case study. The proposed framework consists of the four aspects of business model, concerning customer value proposition, key resource, key process and profit formula. The customer value proposition includes functional value, hedonic value, contextual value, relationship value and ethical value. The selected cases are Orpea in Europe, Sompo Care and Nichiigakkan in Japan, and Suang-Lien Elderly Center in Taiwan. This study collected the strategic events of services and business models from 2000 to 2021 for each case. Moreover, the study conducted in-depth interviews. The findings are described as follows. Firstly, the cases can focus on and balance the development of the five value propositions of customers. They initially pay more attention to the functional value of care and the contextual value created by a friendly environment. In the mid-term, they develop towards ethical and hedonic values. In the near future, they focus on the relationship value between residents and their families. Secondly, in the aspect of key resource, most of the initial investment are focused on in the infrastructure of long-term care institutions and the integration of cross-disciplinary human resources. In the mid-term, the cases move towards integrating with external resources. In the near future, they focus on investing in the resources of emerging technology. Thirdly, in the aspect of key process, the cases place initial emphasis on the establishment and maintenance process of the institutional environment, the resident care process, and the employee training process, etc. In the mid-term, they focus on improving the quality of care process. In the near future, they move towards emphasizing the relationship value process, new technology care process and the process of cooperating with external companies. Finally, in the aspect of profit formula, the cases place initial emphasis on continuous improvement of care services and environment. In the medium-term, they focus on care methods and the creation of a happy environment. In the near future, they move towards emphasizing the residents’ health, safety, and psychological living environment. Furthermore, they place more emphasis on the new care type between “people” and “technology.”
