本文在探討照護一位情感型精神分裂症患者，因服藥遵囑性差而第四次住院的護理經驗，護理期間為98年10月1日~98年11月9日，筆者依身體、情緒、智能、社會、靈性等五個層面進行護理評估，歸納個案主要護理問題有思考過程改變、知識缺失與家庭因應能力失調。護理過程中，筆者藉由傳遞真誠、接納、關懷的態度，與個案建立良好護病關係，強化個案的服藥動機，與其服藥行為連結；並藉由「出院準備服務」之持續性照顧，護理期間為98年11月10日~98年11月27日，以電話訪談及居家訪視方式，提供個案及家屬出院後持續性照護，可達成的照護成果包括:個案能規則服藥、定期至醫院門診追蹤，且明顯提昇遵囑服藥行為與疾病管理的能力。因此，期望藉此護理經驗分享，提供其他護理人員在照護精神病患不遵囑服藥的相關問題。

This article discribed the nursing experience of caring a medication-noncompliant schizoaffective patient who was re-hospitalized for four times. The whole nursing period was from Oct. 01, 2009 to Nov. 27, 2009. Nursing assessment encompassed five levels, including physical, emotional, mental, social, and spiritual aspects. The following three major health problems were identified: altered thought processes, inadequate knowledge and ineffective family coping. During the nursing process, the author delivered her sincere, accepting, and caring attitude, established a good nurse-patient relationship, and strengthened client’s motivation for taking medication. Discharge planning service, consisted of telephone and home visits, was applied to support client and his family to regularly take medication and have OPD appointments under the continuous care delivery. Client’s medication adherence and improved illness management were significantly enhanced. Therefore, this article expects to share with mental healthcare nurses and other professionals the caring experience related to the medication-noncompliance issues.
