題名:老年巴金森患者之保健食品利用--以北部醫院門診患者為例

背景:近年來,許多研究都曾檢視台灣民眾,尤其是中老年人利用保健食品之情況與相關因素。巴金森氏症乃一種慢性中樞神經系統退化疾病,疾病盛行率隨年齡增長而增加,病期長且易出現其他慢性疾病。然而,針對此群體之保健食品利用調查卻十分匱乏。目的:以Andersen醫療利用行為模式為基礎架構,加入認知與態度等心理變項,用以探討老年巴金森氏症患者之保健食品利用與相關因素。方法:研究對象為65歲及以上之老年巴金森氏症門診病患。收案地點為台北市與新北市四所醫院神經內科門診部。研究工具為自行發展編制之結構化問卷。資料檔以SPSS16.0版進行描述性統計與邏輯斯迴歸分析。結果:有效樣本共計200人(男104人,女96人。平均年齡:75.07±7.47歲)。近六成受訪者(57%,n=114,其中男性佔59.6%,女性則佔40.4%)表示受訪前一個月內曾服用過保健食品。邏輯斯迴歸之結果顯示:罹患其他慢性病(共病)、性別(女性)與疾病認知為影響此群體利用保健食品之顯著變項。結論:呼應以往之研究,本調查發現健康狀況與性別的確會影響老年巴金森患者之保健食品利用。另外,對疾病的認知也在此議題上扮演舉足輕重的角色。因此,建議醫院提供病患與其照顧者相關之衛教知識,且應繼續探討心理變項對醫療照顧利用之影響。
Objectives: Based on a modified Andersen behavioral model of health services use with additional psychological variables such as attitudes towards health care services and disease-related awareness, this study aimed to investigate the utilization of dietary supplements among older ambulatory patients with Parkinson’s disease (PD). Methods: Older PD patients (≧65 years) were recruited from 4 out-patient departments of neurology in the Taipei area. Face-to-face interviews were carried out on the basis of a structured questionnaire. A set of significant variables were entered hierarchically into logistic regression models to determine risk factors affecting the utilization of dietary supplements among the participants. Results: With an average age of 75.07±7.47 years, the sample comprised 200 participants of whom 96 were female and 104 male. 57% disclosed the usage of dietary supplements within a one- month-period before the current study. The logistic regression analysis indicated co-morbidity, gender (female) and disease-related knowledge as significant risk factors for the utilization of dietary supplements among older ambulatory Parkinson patients. Conclusion: Older PD cases having more than one chronic disease besides PD, with long disease duration were found to be more likely to utilize dietary supplements. Specifically, disease-related knowledge was identified as significant as well. Out-patient clinics are recommended to offer this target group and their care givers education programs specifically those regarding dietary supplements. The future research is suggested to explore potential impacts derived from psychological variables on the issue examined.
