題名:臺北市國小學童雙親代謝疾病及體能活動與身體質量指數之關連

孩童肥胖已逐漸成為全球性的健康問題,這個問題的重要性在於成人肥胖和慢性疾病罹患率與孩童時期的肥胖程度之間具有顯著的關係。本研究針對臺北市14,560名國小六年級學童身體質量指數(body mass index, BMI)進行回溯性追蹤研究,探討其雙親代謝異常情況及校園體能活動參與程度與過去六年間BMI成長的關連性。本研究分為兩個子題:第一個子題主要的研究目的為調查父母患有代謝異常情況的學童在戶外運動與靜態活動的選擇,以及過去六年BMI成長趨勢是否與其他學童有明顯差異。子題一研究結果發現,父或母已診斷為糖尿病、心臟病、中風、高血壓者,其學童BMI之基線值(baseline)與成長速度均明顯高於其他學童。另外,這些學童花費於靜態活動例如電腦使用與聽音樂時間較長,參與戶外運動的時間較短。第二個子題主要研究目的為調查參與校園體能活動程度(運動代表校隊 v.s 運動性社團 v.s 未參加學校運動性組織)與國小學童BMI成長趨勢的相關性。子題二研究結果發現國小學童不論男性或女性參加運動校隊者其BMI成長趨勢明顯較低。結論:本研究的主要發現顯示參與運動社團無法對於BMI成長趨勢有顯著的抑制效果。另外,學童參與運動的傾向與家庭慢性病遺傳因素有關。因此學校應將父母已具有代謝異常情況的學童視為重點,增強其運動參與;推廣校隊可作為協助孩童肥胖控制的有效方法。
Childhood obesity has becoming a global health problem. This problem is important since the relative risks for adulthood obesity and metabolic syndrome are closely associated with body mass index (BMI) in childhood. In the study, we retrospectively tracked BMI from 14,560 schoolchildren in Taipei City. In the first part of the study, we determined the link between metabolic disease condition of parents and 6-year BMI progression of children, in conjunction with their physical activity status. The result shows that the children with parents having diabetes, coronary heart disease, stroke, and hypertension, had significantly greater baseline BMI value (first grade) and BMI increase rate across 6 years against their normal counterparts. In addition, those children with parental metabolic conditions spent more time in computer use and less time in outdoor physical activity. In the second part of the study, we compared the 6-year BMI progression for schoolchildren participating in sports club, team sports, and the rest of students. Schoolchildren participating in team sports has lower baseline BMI. Interestingly, we found that these children had substantially lower increased rate in BMI between 4th and 6th grades. No significant difference in BMI increase was found between children participating sports club and the rest of sedentary students. Conclusion: Schoolchildren with parental metabolic diseases should be regard as a special group for early obesity prevention by correcting their sedentary lifestyle. However, we must note that participation in sports club is not sufficient to suppress BMI increase for schoolchildren. Participating more vigorous sport team in campus would be an effective method to suppress the development of obesity for schoolchildren.
