本文敘述協助一位老年住民適應遷住養護機構的護理經驗○個案因身體功能退化且家人無力照顧，所以遷住機構。個案因無法適應機構制式作息，以及缺乏個人隱私，進而產生睡眠障礙、焦慮、低自尊和疾病加重等情形，筆者確認個案有「遷移壓力症候群」的護理問題。經由治療性人際關係建立，引導個案說出其需要及想法，協助建立正向因應策略及支持系統，進而促進其適應機構的生活。

　　This article describes a nursing experience of assisting an elder resident adjusting into a residential facility. This case moved to the nursing facility because of physical impairment and families’ inability to provide care. Due to mal-adaptation to the rigid life pattern and lack of privacy, this case had problems of sleep disturbance, anxiety, low self-esteem, and worsened disease conditions while living in the facility. He was diagnosed with the health problem of “Relocation Stress syndrome.” By establishing the therapeutic interpersonal relationship to elicit the case to express her needs and thoughts, it could subsequently help the case to develop adjustment strategies and support systems to adjust her life in the residential facility.
