題名:自閉症兒童對步態異常之探討

背景:自閉症所伴隨之動作障礙可能會抑制其他動作技巧的出現或限制動作能力的發展,因而導致自閉症在日常功能性的動作產生不穩定或異常,例如步行。研究發現異常的步態可能會導致疼痛、疲勞、關節額外受壓迫並影響到日常生活中功能性的動作而導致生活品質明顯下降。過去多使用標準化測驗評量或影像紀錄評估自閉症患者的步行動作,雖然發現異常步態,但方法可能過於主觀且多著重在嬰幼兒或學步時期的研究。近年來採用生物力學量化分析方式特別針對自閉症兒童時期的步態表現進行的研究有增加的趨勢,對於自閉症兒童的異常步態表現的探討越來越受重視。方法:收集國內外有關自閉症兒童步態表現相關之相關文獻,以文獻回顧之方式加以探討。結語:結果發現在運動學方面,分析發現不同學者使用不同的研究方法所產生的結果並不完全一致,例如步幅、步頻或站立期時間及下肢關節角度等。在動力學方面,雖在下肢關節力矩的研究中發現差異,但相關文獻仍較為不足,無法證實與正常兒童間存在那些差異及不能更深入探討到自閉症兒童下肢受力負荷及肌群作用情形。因此對自閉症兒童步態的異常表現仍需投入更多的研究進行探討。
