目的:越來越多醫院採用電子病歷以改善醫療服務品質,然病歷由紙本改為電子方式,對於病歷管理人員的影響尚不明確,本論文主要目的為探討電子病歷所帶來對科技壓力與病歷人員工作疲憊的關聯,及此種工作疲憊和病歷人員工作表現的關係。方法:透過問卷調查法,以南部醫學中心與區域醫院病歷人員為調查對象,總共蒐集218份有效資料,所蒐集資料透過結構方程模式進行分析。結果:資料分析結果顯示工作超載、入侵效應與複雜度的確造成病歷人員工作疲憊,此種工作疲憊亦會降低病歷管理人員工作表現,而區域醫院在工作超載和工作疲憊的關聯強度顯著高於醫學中心。結論:依據研究發現,本論文建議各醫院應注意電子病歷對於病歷人員所產生的科技壓力問題,並擬定因應策略,藉以減少此種壓力對於病歷人員可能產生的負面效應,確保醫院電子病歷的正常運作。

Objectives: An increasing number of hospitals are adopting electronic medical records in order to improve healthcare quality. The potential influences of transforming paper-based medical records into electronic medical records on health information management staff are however still not well understood. This study aims to investigate the relationship of techno-stress arose from electronic medical records with work exhaustion, and also the association of work exhaustion with work performance of health information management staff. Methods: Survey methodology was used to collect data from health information management staff of medical centers and regional hospitals in southern Taiwan. Totally, 218 valid questionnaires were gathered and analyzed by structural equation modeling. Results: The results reveal that techno-overload, techno-invasion, and techno-complexity predict work exhaustion, and work exhaustion, in its turn, predicts work performance. Further, regional hospitals demonstrate a significant stronger effect of techno-overload on work exhaustion than medical centers. Conclusions: Based on the findings, this study suggests that hospitals should pay attention to the plausible negative influences of techno-stress, caused by electronic medical records, on health information management staff. Appropriate strategies should be proposed to lower the downside effects of electronic medical records on health information management staff, and to secure normal operation of electronic medical records.
