本研究採質性研究方法，目的在了解台灣地區發生手部職業創傷載件患者之創傷後反應並發展該評估量表以提供未來醫護人員使用。樣本來自台北某家醫學中心以及一家區域醫院，以診斷為手部職業外傷患者，且術後在三日以上、一年以內，傷殘嚴重程度符合勞保殘障給付之中度至重度(2-9級)為收案業象。樣本數採資料飽和原則，運用半結構式訪談指引共訪談11名個案，錄音訪談內容經轉錄謄寫後，再經內容分析，其內在信度為0.97，外在信度為0.92。所得資料補區分為:生理的、情感的、認知的、行為的四大類之創傷後反應，共有25個反應類目，其臀以「焦慮」為最多，並依研究結果及經專家效度測試發展一42題之量表。未來建議採大樣本做內部一致性、再測信度、同時效度以及建構效度之檢定，以建立「手部職業外傷患者之創傷後反應評量表」的信、效度，使臨床工作人員能迅速總握患者之心理狀況，提供整體性護理措施。

　　　　　 A qualitative method was used to investigate the post-trauma response of occupational hand injuries in Taiwan and develop a post-trauma response inventory (PTRI). Subjects were chosen from a medical center and a regional hospital. Criteria for sampling were hospitalized patients that have been diagnosed with a work-related hand injury, post-operation of three days to one year, and meet the 2nd to 9th categories of disabled level for disable reimbursement by labor insurance. Sample size was determined by the saturation principle. Eleven subjects were interviewed in depth using a semi-structured interview guide. The interview data were tape-recorded and then transcribed. Content analysis was used to analyze the interview data. The intra-rater reliability was .97, while inter-rater reliability was.92. The results were reclustered into four dimensions of post-trauma response such as physiological, affective, cognitive, and behavioral. Under the four dimensions and 25 categories of post-trauma response and the most experienced post-trauma response associated with these patients was "anxiety". The result yielded a 42-item PTR. Further study is recommended to test internal consistency, test-retest reliability, concurrent validity and construct validity by large samples in order to determine the reliability and validity of PTRI and help health professionals to assess subjects' traumatic reaction to provide appropriate intervention as soon as possible.
