題名:身體受虐兒童遊戲治療中的遊戲行為之分析研究

本研究的目的在針對身體受虐兒童遊戲治療中遊戲行為的表現及其變化做初探性研究。本研究採取自然主義的研究取向及質性的分析方法,由研究者擔任遊戲治療中的治療者,分別就五歲、七歲、與八歲的三位身體受虐兒童,以他們在22、13與15次不等的遊戲治療中之遊戲行為表現與改變進行個案分析。研究發現如下:
1.從治療的過程來看,本研究的三位身體受虐兒童在遊戲治療中,都各自有不同的治療進展及行為型態,亦呈現不同的個人主題。
2.在人際關係上,本研究之身體受虐兒童普遍的困難是無法以合宜的方式與人建立關係,而以退縮或攻擊兩種不同型態來測試關係。以退縮型態建立關係的孩子,其想像遊戲及表達內容均顯得單調、貧乏而抑制,害怕觸及個人議題;而以攻擊型態測試關係的孩子,則想像遊戲及表達內容均很豐富,且能運用攻擊想像遊戲,來處理無法言說的受虐經驗。
3.身體受虐兒童在面臨重大壓力時,傾向主動以攻擊或被動攻擊的行為來保護自己免於傷害或挫敗的情境。而在其想像遊戲中,攻擊主題亦是相當常見而重要的主題,對男生尤然。
4.隨著治療關係的進展,測試關係的行為率先減少,而其他向度的遊戲行為變化情形,則會因個人議題而有所不同,不只環繞單一主軸發展。不過,治療關係的發展則為共同主軸,而帶動其他向度的改變,最常見的是會促進表達的增加。
5.在治療階段的進展上,分別出現直線發展模式與循環發展模式。之所以會出現循環的發展模式,可能與治療週數較長,或生活中重要事件的影響有關;除此之外,治療中表達的內在衝突或情感過於痛苦,而必須藉著退回,並再度考驗關係以保護自我,亦可能為關鍵因素之一。
研究者並針對上述研究結果加以討論,並據此分別對實務工作者及未來研究兩方面提出具體建議。
The purpose of this research was to chronicle the change of play behaviors during play therapy with physical abused children. Naturalistic approach and qualitative analysis were employed, the researcher took the role of both therapist and researcher. Three physical abused children whose age were five, seven and eight year old receive 22, 13, and 15 weekly play therapy sessions. A case study were utilized to present the patterns and change of play behaviors.
The following findings were induced :
1. The progression of play therapy and the pattern of play behaviors were highly individualized.
2. The children were not able to relate with others appropriately. They tested the therapeutic relationship in two ways: withdraw or aggression. For the children with the style of withdraw, their fantasy play and expression were monotonous, poor, and inhibited, and they avoided to divulge any information about abused history. For the child with the style of aggression, their fantasy play and expression were rich, and he worked through the inhibited abused experience by aggression in fantasy stories.
3. Under the high pressure, the physical abused children inclined to cope with active aggression or passive aggression. Their typical and important theme in fantasy play was aggression, especially for boys.
4. When rapport was established, the testing behaviors of relationship declined, other play behaviors would change in accordance with personal need. However, therapeutic relationship seemed to be the center of change process, and changes in other dimensions especially for self-expression were subject to relationship issues.
5. The course of treatment proceeded in linear or circle trend. The factors affecting the style of progression might contain: (1) the duration of the play therapy, (2) the effects of significant life events, and (3) the fears of their expressed conflict and feeling and subsequent regression and testing for protection from self-hurt.
The results of comparison and analysis were discussed. Based on the discussion, practical suggestions and applications were made for counseling practician and future research.
