本篇個案報告是敘述一位48 歲男性因頸椎動靜脈畸形接受栓塞手術失敗導致呼吸器依賴病人之護理經驗，護理期間為2018 年04 月17 日到2018 年05 月15 日，運用 Gordon 十一項健康功能型態評估作為護理評估工具，與個案及家屬開放性會談、利用觀察及身體評估，收集主客觀資料，確立主要護理問題為：呼吸道清除功能失效、知識缺失、焦慮。護理過程中個案因需再次面臨氣切手術而擔心風險及併發症，筆者運用實體立體創意海報的衛生教育方式，提供氣切相關資訊，成功使呼吸器依賴個案接受氣切手術，並降低對氣切術前焦慮，達到身心最佳調適狀態。

This case report is a 48-year-old man who suffered from Ventilator- dependent due to failure of embolization surgery due to cervical arteriovenous malformation.The nursing period is from April 17, 2018 to May 15, 2018. The author uses Gordon’s 11 Functional Health as a nursing assessment tool, open interviews with individual cases and family members and use observation and physical assessment to collect subjective and objective. The data was analyzed and summarized into three nursing problems : Ineffective airway clearance, lack of self-care ability, anxiety. During the nursing process, the patient is faced with worried about risks and complications due to the need for tracheotomy surgery again. The author uses the health education method of the entity stereo creative poster to providing information on tracheotomy, successfully making the respirator dependent on the case of tracheotomy surgery, and reducing the anxiety before the tracheotomy surgery, to achieve the best adjustment of body and mind.
