題名:注意力缺陷過動症中文版Swanson, Nolan, and Pelham, Version IV (SNAP-IV) 量表之常模及信效度

目的:建立台灣地區父母版和老師版SNAP-IV 量表之常模與其中文版信效度。方法:在台北、桃園、嘉義、台南四個地區,選取六所國小與六所國中學童(一至八年級),由父母填寫中文版SNAP-IV 及CBCL,老師則填寫中文版SNAP-IV 量表。總計父母版有效問卷為3534 份,老師版為3653 份。間隔一個月後以隨機方式選取父母221 名與老師233 名進行再測信度施測。結果:父母版SNAP-IV 的不專心、過動和對立反抗分量表的再測信度(ICC) 分別為0.72、0.67 和0.59。老師版的三個分量表之再測信度良好(ICC = 0.60-0.84)。父母版與老師版SNAP-IV 各分量表之內在一致性高(所有Cronbach's ≧0.88)。中文版SNAP-IV 三分量表和CBCL 的相似行為分量表之相關係數高(r = 0.51-0.72)。各年級的三分量表得分,男生均顯著地比女生高。預估的ADHD 盛行率為 7.09%-12.04%。結論:中文版SNAP-IV 量表是一個具有良好信效度的量表,可協助臨床評估與診斷注意力缺陷過動症,以及作為治療成效的評估工具,其也可作為社區或學校篩選之用,而且本研究建立的量表常模同時具有跨種族及文化意涵。
Objective: To develop the norms and to test the psychometric properties of the Chinese version of the Swanson, Nolan and Pehlam version IV (SNAP-IV) scale-parent and teacher forms. The prevalence of attention-deficit/hyperactivity disorder (AD/HD) was also estimated. Methods: A school-based sample from the first to the eighth grade was recruited from the following four areas of Taiwan: Taipei, Taoyuan, Chiayi, and Tainan City. Participants' parents (n=3,543) completed the Chinese SNAP-IV-parent form, and CBCL scales while teachers (n=3,653) completed the Chinese SNAP-IV-teacher form. The test-retest reliability study was performed using randomly selected parents (n=221) and teachers (n=233) . Results: Intraclass correlation coefficients for the three subscales of the Chinese SNAP-IV ranged from 0.59 to 0.72 for the parent form and from 0.60 to 0.84 for the teacher form. All subscales of both the parent and teacher forms showed excellent internal consistency with Cronbach's greater than 0.88. The concurrent validity of the Chinese SNAP-IV was satisfactory. Boys were scored significantly higher than girls in the three subscales across the 8 school grade levels in both the parent and teacher forms. The estimated prevalence of ADHD based on the three approaches (P95, Tscore >70, and DSM-IV criteria) ranged from 7.09% to 12.04%. Conclusion: The results of this study suggest the Chinese SNAP-IV scale is a reliable and valid instrument to measure core symptoms of ADHD for clinical and research purposes and to support its use in schools and the community for screening in Taiwan.
