本研究旨在探討居家照護期間居家個案肌力訓練衛教後，實驗組與對照組居家個案肌力改變之成效。採類實驗法兩組前後測設計，研究對象為某醫學中心居家護理服務之個案，以立意取樣方式採收案號次序分派為實驗組與對照組，共有53位個案完成訓練計劃，其中實驗組27人，對照組26人。而實驗組由物理治療師評估與指導執行跨專業居家物理治療衛教介入，並由居家護理師每週電訪2次、家訪時追蹤家屬執行肌力訓練，對照組僅有物理治療師評估無復健運動指導，居家護理師亦無電訪及家訪追蹤肌力訓練，45天後分別再進行兩組四肢肌力評估。實驗組與對照組無論在性別、年齡、意識狀態、罹患慢性病數基本屬性以卡方檢定比較無顯著差異。經不同的介入措施後，以Wilcoxon signed ranks test檢定，對照組肌力除左上肢肌力在統計上有顯著差異(p<.05)，其他右上肢、右下肢、左下肢皆無顯著差異，實驗組則四肢肌力改善皆有顯著差異(p<.001)。本研究結果建議健保局，能將居家物理治療衛教早日納入居家護理整合性服務項目，以持續維持或增進居家個案肢體肌力，且此研究亦可作為居家護理人員有效提供居家個案肌力衛教之參考。
The purpose of this study was to evaluate the change of muscular strength in home-care cases with and without health education of muscular strength training during the period of home-care. A quasi-experimental two groups pretest-posttest research design was designed as a guide for this study. The subjects are purposive sampling from home-care cases in a medical center. Among 53 subjects who finished the training program, 27 of them were in the experimental group, and 26 of them were in the control group. The experimental group was intervened by a physical therapist giving evaluation and guidance of home-care rehabilitation exercises. The home care nurses called the patients twice a week, and asked their relatives for the status of muscular strength training during home visits. After 45 days, muscular strength of the limbs is measured for both groups. The chi-square test showed that both groups had no difference in basic characteristics such as gender, age, consciousness, and number of chronic diseases. Two groups data of muscular strength were analyzed by Wilcoxon signed ranks. There was a statistically significant difference in left upper limb in control group (p<.05) and was statistically significant difference in both upper and lower limbs in experimental group (p<.001). The results as a reference of health education of muscular strength for home care cases.
