題名:農村地區銀髮族對遠距健康照護系統持續使用意圖之研究

面對世界人口老化的趨勢,我國自然無法置身事外,目前我國已正式進入了高齡化社會並即將邁入高齡社會。尤其農村地區,老化問題更加研究。為達成「在地老化」為政策目標,應用遠距健康照護服務來滿足日益增加的長期照護需求已成為選項之一,然而目前卻少有對於農村地區的的老人對於遠距健康照護系統使用行為的相關研究。因此,本研究透過先質性後量化的方式進行研究,以已參與竹山秀傳醫院遠距照護服務的農村老人為研究對象。首先進行文獻探討探索老年人對於遠距健康照護系統持續使用之因素初步步建立假說;再透過質性訪談加以確認;最後建立研究模型透過量化方式進行驗證。
經過分析,本研究發現在遠距健康照護系統的使用行為上,知覺有用性對於他們的持續使用意圖並無直接影響效果,而是被滿意度所完全中介。但這樣的結果與質性訪談一致,亦即他們對於遠距健康照護系統是否有助於他們的健康是存疑的。遠距健康照護系統並非遠距醫療系統,對於年長者而言,他們並無法立即獲得或者感覺到遠距健康照護系統所帶來的成效,而透過醫院健健管師的協助與投入,強化了老人們對於遠距照護服務的滿意度,進而讓他們持續使用。第二個重要的研究發現是老年人的受嘲恐懼感知會影響他們對於遠距健康照護系統的使用意圖,他們害怕個人的生理狀態不佳而受到同儕取笑,故不願持續使用遠距健康照護系統。但可透過社會支持感知進行調節。
本研究驗證了資訊系統接受後持續使用模型應用在農村老人使用遠距健康照護服務中的可行性,除了對於農村老人此一特殊樣本使用行為的深入研究外,並將遠距健康照護系統使用的研究領域從初始接受拓展至持續使用。此外,本研究亦可提供參考給想要導入遠距健康照護服務的地方政府、企業,並讓他們得以依據研究成果加以修正與調整其推動策略。
Faced with the trend of world aging, Taiwan will surely stands in its position instead of staying out. Now, our country has stepped into the rode of population aging and will march towards an aging society, which is particularly apparent in rural areas, where research on this matter is demanded. To attain policy goal of aging in place, Tele-healthcare service provided to satisfy the growing need of long-term care has fallen into items of selected method. Yet, there is few study on utilization of Tele-healthcare system by the aging in rural areas, which can provide guidance on our study, that is qualitative data prior to quantification. Taking elderly people receiving service of Tele-healthcare in Chushan Show-Chwan Hospital as the object of study, firstly to figure out starting point of continuous usage of Tele-healthcare system through related documents, then to build up preliminary hypothesis and confirm correctness of the hypothesis by substantive interview. Finally, to build up a research model to perform verification by quantification method.
The analysis result of research shows that as to usage of Tele-healthcare system, perceived usefulness has no direct effect on continuous usage intention; instead it is impacted by satisfaction. The result is in line with findings in substantive interview, that is there are still questions on whether Tele-healthcare system will bring health benefits. Tele-healthcare system is different from telemedicine system in that the old people can’t experience or perceive effectiveness of Tele-healthcare system immediately. While with assistance and input of health management accelerator, the old are becoming satisfactory with the system, which promotes continuous usage. The second research finding is that Gelotophobia of the elderly has impact on their desire for continuously utilizing of remote health care system. They are afraid to be made fun of by peers due to their personal physiological state, so that they would not continuously utilize the remote health care system. However, it can be adjusted by social support perception.
This research verifies feasibility of appliance of A post-acceptance model of IS continuance on utilizing remote health care service by the elderly in rural region. Except using deep behavioral research on the special sample of the elderly in rural region, we expand research region of remote health care system from initial acceptance to continuous using. Moreover, this research also provides reference for local governments and enterprises who desire to bring in remote health care, and help them revise and adjust promoting policy based on research results.
