2004年2月4日行政院所通過的「社區營造條例」草案，為挑戰2008的國家施政方針訂定了方向，也賦予社區更為積極的自主能力，願意以更為積極的態度尊重社區居民認同形塑的價值，和據此價值所產生出集體行動的結果。本研究利用過去一年的時間，結合整體的文化認同形塑和特殊的社區健康營造，以輔導的方式建立涵蓋全國的支援網絡，將結合在地文化重建的社區健康營造診斷概念導入社區，促進整體社區認同形塑。以此為基礎，我們提出了三個社區健康營造的模式，作為後續發展健康生活社區化政策的基礎。
New national policy on community building has valued the approach of empowerment, the autonomy of local community, the importance of new identify formation as well as those following collective actions. To follow this policy, this one year research project has taken nationally wide on-site research to link the discourse on cultural identity formation and the discourse on community health building. During the time period, a mutual supportive network capable of empowering various local community health building efforts has been established. Our network, as a research team, combines an oral historical approach and the empowerment-oriented community health building strategy. We attempt to facilitate the linkage between cultural identity formation and healthy lives engagement in various local communities. Accordingly, this paper proposes three practical models to transform current community health building strategies to new national policy.
