題名:醫院血液透析業務採行之策略型態與績效間關係之探討:競爭強度的干擾角色

本研究旨在探討醫院血液透析業務採取不同策略型態(垂直整合、合作與委外經營)對績效(效率與品質)之影響,同時也驗證在情境變項(區域競爭強度)的干擾情況下,是否會調節策略與效率及品質之間的關係,以交易成本理論與動態競爭觀點分別論述變項之間可能的關係與結果。
以2012年與2013年經行政院衛生福利部公告醫院評鑑層級為「區域醫院」與「地區醫院」之辦理血液透析業務醫院為研究對象,分別了解其經營策略型態與績效(效率、品質)及競爭強度相關資料。本研究實證結果發現,在其它條件不變下,採行合作策略較垂直整合策略有較高的效率,而採行外包策略較合作策略與垂直整合策略有較高的品質;在交互作用部分,區域競爭強度會負向調節合作策略與效率之間的關係,而區域競爭強度也會正向調節垂直整合策略與品質之間的關係。
本研究之貢獻在提供有助於血液透析業務效率與品質提升之策略型態建議,同時以區域競爭強度當情境變項以衡量是否有不同的影響結果。
This paper discusses the influence of strategic types (such as vertical integration, cooperation, and outsourcing) on performance (such as efficiency and quality) with respect to hospitals'' hemodialysis business. Additionally, this paper verifies whether the relationship between strategic types and efficiency/quality will be regulated under situational moderating variables (such as regional competitive intensity), and discusses the possible relationship between these variables, from the perspectives of transaction cost theory and competitive dynamics.
The study covers hospitals that provided hemodialysis business to the public and that were rated as “regional” or “district-level” hospitals by the Ministry of Health and Welfare of Executive Yuan in 2012 and 2013. It investigates their operational strategic types, operational performance (including efficiency and quality), and competitive intensity. The empirical findings may be summarized as follows: 1) all other factors remaining unchanged, the cooperation strategy will result in higher efficiency than the vertical integration strategy while the outsourcing strategy will result in higher quality than the cooperation and vertical integration strategies; 2) in terms of interaction, regional competitive intensity will negatively regulate the relationship between the cooperation strategy and efficiency, but will positively regulate the relationship between the vertical integration strategy and quality.
Accordingly, this paper offers suggestions on the strategic types that serves to improve the efficiency and quality of hemodialysis business, and uses regional competitive intensity as a situational variable to assess whether any different influence may be produced.
