本研究目的旨在探討青少男異常飲食發展歷程,與相關之男性經驗內涵。研究者以生態系統概念切入,藉由詮釋現象學方法論觀點,試圖還原飲食異常青少男之經驗本質,並詮釋經驗蘊含之深層意義。研究過程採用二階段研究設計,於台南市立意選取菁英高中男校一所,以「飲食態度量表—26題」(EAT-26)對當時高一、高二全體男生共1800位,進行第一階段初步篩選,隨後再針對所篩選出之73位青少男高危險群,進行第二階段診斷性晤談,最後邀請到七位符合本研究條件者,進行二至七次之深入訪談,並以「主題分析法」對所蒐集到之三十一份文本資料,進行分析。
研究結果主要發現如下:1. 參與者浮現六項以「吃」貫穿之經驗主題,呈現青少男飲食異常發展歷程與男性經驗:(1)「愛吃—期待又怕受傷害」;(2)「恨吃—決心減重振聲威」;(3)「拒吃—受苦只盼外型佳」;(4)「愧吃—催吐抗吃心矛盾」;(5)「困吃—提心吊膽怕發胖」;與 (6)「難吃—愛美中性男難為」。2. 與過去研究結果相較,具有飲食異常問題之青少男與青少女,顯現十分相似之生活經驗與脈絡,然身為社會文化建構下之男性,仍具有某些男性特有之經驗,而由性別之角度分析,傳統男性性別角色期待似保護了大部分男性,不致落入飲食異常之處境,但對於已成為飲食異常患者的男性而言,此對性別期待亦同時成為發病後的惡化因子,本研究之參與者即因不符傳統男性性別角色,造成不合模的處境,而未能受到男性保護因子眷顧,致落入飲食異常處境,且使得疾病歷程更加隱晦、激烈與無助。本文最後針對男性飲食異常患者之處境,提出對未來研究與諮商實務、教育者之建議與省思。
The purpose of this study is to explore the developmental process of eating disorders and related male experiences. In order to understand participants’ experiences and the profoundly hidden meanings of this disease, the concept of ecological system was applied and hermeneutic phenomenological methodology was used in the study. Two–stage design was used. Stage one: “Eating Attitude Test-26” (EAT-26) scale was administered to 1800 male students in grade 10 and 11 from one elite high school in Tainan. Stage two: After interviewing 73 adolescent males who were identified as having abnormal eating attitudes by using EAT-26, seven boys who fit in the criteria of this study were selected. Two to seven in-depth qualitative interviews were conducted with each boy and thirty-one transcripts were obtained. Thematic analysis was used to analyze the data.
Two major findings were identified: 1. Regarding participants’ illness process of eating disorders and male experiences, six essential themes were generated: (1)”like to eat- look forward to but afraid of getting hurt”; (2)”hate to eat- decide to lose weight for getting good reputation”; (3)”refuse to eat- hope to get good shape although it might be suffering”; (4)”be ashamed to eat- vomit it out against over eating but feel ambivalence”; (5)”puzzle to eat- afraid to eat for getting fat”; and (6)”difficult to eat- the sissy boys like to dress up and make up, easily excluded by others. 2. Compared with previous studies, teenage boys and girls with eating disorders appeared similar living experiences and context. However, there were some male-specific experiences: male sexuality seemed to protect most teenage boys from eating disorders and on the other hand male sexuality worsened eating disorders after onset. Most participants in this research did not fit in traditional gender roles for males and were not protected by their male sexuality, which tended to develop eating disorders. In the discussion session, future works for studies, counseling, and education for eating disorder male patients were proposed.
