長期障礙者老化已成為一新興的社會現象,國外對此議題有較多面向的探討,國內則仍欠缺累積相關文獻。本研究運用現象學方法,探討中高齡長期障礙者的老化經驗與老年生活期待。研究參與者共廿九位,女性十五位,男性十四位,年齡最低四十三歲,最高五十九歲。研究者依生命歷程經驗將之分為三群組,包括終生認知障礙的「永遠的孩子們」、終生生理障礙的「一路走來始終如一」、以及在青少年或成年以後發生生理或認知障礙的「生命大逆轉」。研究結果以研究參與者「家庭生活」與「療癒照顧」二面向描述其生命歷程的經驗。研究者認為,對於中高齡長期障礙者的老化經驗與老年生活期待的本質探究,必須回到身心障礙經驗的根源、身心障礙者在世存有的人我關係、身體寓居於世的生活環境、以及生命歷程一連串的安排與選擇來耙梳出經驗的脈絡,研究者最後歸納出調和老化模型,作為認識中高齡長期障礙者老化經驗與老年生活期待的架構。研究結論:中高齡長期障礙者具有共同與獨特的生命歷程,其障礙經驗是在單一身體形象的社會中建構的,老化經驗是藉由身體與生活環境互動而累積,因而具有世代差異與個別屬性差異,老化過程會經驗到醫療科技的有限與發展自我療癒照顧,老年生活是有區隔的選擇與道德安排。調和老化模型的觀點可以強化並補充當代老化與障礙觀點的不足,並以此模型檢視中高齡長期障礙者生命歷程經驗到的排除與撤退。研究者對長期障礙者社會福利政策規劃與社會工作實務發展建議:在理念方面,應肯定身心障礙者的主體經驗與公民參與權利,社會政策與福利服務規劃應回歸需求導向的核心價值;在政策方面,建構無障礙的人文態度與物理空間環境,重視服務使用者的參與決定權與選擇權;在社會工作教育與實務方面,社會工作教育應包含跨界的知識及對生命本體的認識、服務輸送過程應落實福利宣導與案主知後自決、發展以人為本的療癒照顧與健康照顧管理計畫,以及發展小單位、具區隔的、似家的機構照顧模式。
People aging with a lifelong disability are a relatively new social phenomena. Many dimensions of this agenda have been discussed in western countries, but not the case of Taiwan. This thesis employs phenomenology to investigate the aging experiences and later life expectations of middle-aged people with a lifelong disability. A total of 29 participants aged 43 to 59 divided into 3 groups by life courses, lifelong cognitive impairment "the forever little child", lifelong physical impairment "the only one way to go", and "the great reversal of life". Results show participants' life course experiences of family life and healing and health care experiences. Researcher suggest that inquire the essences of aging experiences and later life expectations of middle-aged people with a lifelong disability should inquire the contexts of the origin of disability, the I-we relationships of being-in-the-world for people with disability, the living circumstances of fitting-in-the-world for bodies, and the arrangements and choices made during life courses. Researcher proposes the harmonious aging model to construct the knowledge bases of aging experiences and later life expectations of people with a lifelong disability. Conclusion: people with a lifelong disability have common and unique life courses. Disability experiences are constructed by mono-bodyimaged society. Aging experiences are accumulated by interaction of bodies and living circumstances and differ from cohorts and individuals. Experiencing aging will also experience medical technology limitation and develope self-healing and care methods. Later life arrangement is a differential and moral arrangement. Harmonious aging model can strengthen contemporary aging and disability perspectives, and examine the experienced exclusion and disengagement of middle-aged people with a lifelong disability. Suggestion: conceptually, the social policy makers should recognize the subjective experiences and the citizen rights of different groups, reconfirm the core values of need-oriented welfare policy and services. In policy, researcher proposes to construct barrier-free humanity attitude environment and physical spaces, and to emphasize the decision making and choice rights of service users. In social work education and practition, social work education should include the knowledge of different disciplines and ontology of life, services providers should strengthen welfare promotion and informed self-determination, develope humanity healing and health care management plans, and creat small unit, separated, and home-like institutional care models.
