Purpose: To construct a set of Evaluation Indices for Junior High School Health and Physical Education Teachers (EIJHSHPET). It was hoped that it will provide a future policy reference for schools and educational administration organizations. Methods: The research method adopted self-designed criteria for analyzing questionnaires. The participants included 972 junior high school principals, directors of student affairs, sectional chiefs of physical education, sectional chiefs of hygiene and teachers of health and physical education. The measurements utilized standard statistical methods such as frequency distribution, mean, standard deviation and factor analysis. Results: The results showed that EIJHSHPET included 4 categories, 14 dimensions, and 69 indicators. The results of the mean, between 3.16 and 3.44, highlighted the importance of teacher evaluation indices while standard deviation fell between .41 and .51. The most important indicator within 14 dimensions in this study corresponded to relevant questionnaires, indices and studies in the past. Conclusion: The EIJHSHPET is a well-considered method with high validity and reliability. It will be used for teacher evaluation in the future.
