本研究旨在設計適合諮商師之教育與訓練課程,以提升其處理性議題時的自在程度與意願,並且評估該課程介入對諮商師的「性自在」、「性知識」、「性自我覺察」、「溝通技能」及「與案主討論性議題意願」之實施成效。為達到該目的,本研究共分為三個階段進行:
第一階段:運用文獻對性自在的定義與德懷術等探索性研究,建構提升諮商師性自在訓練方案之教學方式與內容,共22名專家參與本階段之研究,得到11項重要之教學項目,依序為「覺察並檢核自己對性的態度、價值觀、信念、迷思、與性行為」、「諮商師對多樣化的性行為之感受與想法」、「覺察在諮商室中的移情與反移情」、「會使用各種正確的性語言而不覺得尷尬或害怕」、「了解性諮商倫理」、「家庭、社會、文化對諮商師性議題之影響與反思」、「性虐待、性侵害、性暴力、與性騷擾」、「諮商師的自我保護」、「案主性議題實作」、「性別認同」、與「探索諮商師自己各階段的性發展歷程與經驗」。
第二階段:根據德懷術調查結果,利用自我效能理論,設計諮商師性自在訓練方案為8個單元、26小時的具體課程,教學單元依序為「一樣米養百樣人—性的多樣化」、「社會中的性階層與歧視」、「生活中的性語言」、「我就是這樣長大的」、「諮商師,你該怎麼辦」、「倫理大審判」、「享受性福無障礙」與「性諮商技巧與實務」。本階段同時進行成效問卷設計與實施,共8名專家、35名諮商師參與本課程問卷之設計與調查。
第三階段:利用準實驗法實施諮商師性自在訓練方案,並進行成效評估。實驗組31名、控制組31名,共62名諮商師參與本階段,經統計分析結果如下:
一、依據訓練架構編制所設計之「諮商師性自在課程」實施後,對實驗組「性自在」、「性自我覺察」、「溝通技能」與「處理性議題意願」有立即顯著效果。在性知識層面上,絕大部分受試者有顯著差異,但對於原本就是高分者,課程介入後的差異不大。
二、「諮商師性自在課程」實施後對實驗組的「性自在」、「性知識」、「性自我覺察」、「溝通技能」與「處理性議題意願」有顯著的延宕效果。
三、參與本研究之控制組,在性自在、性知識、性自我覺察、溝通技能與處理性議題意願之前後測成績,沒有顯著差異。
四、從「諮商師性自在課程」之過程評量結果顯示,大多數的實驗組都認同、喜歡、且能達到本課程在相關層面之教學目標。
The purpose of this study was to design an appropriate training program for counselors to raise comfort level and willingness when they discuss sexuality with their clients. There were 3 stages in this study:
Literature review and Delphi Technique were used on the first stage to study the topics for the sexual comfort training program for counselors. Eleven essential teaching topics for the program, found by 22 experts, were stated as the followings: (1) being aware and examining one’s own attitude, value, faith, myth and behavior toward sexuality; (2) having adequate knowledge of the various aspects of human sexuality; (3) transference and countertransference in counseling sessions; (4) using accurate language of sexuality; (5) the ethics of sexuality counseling; (6) the influence of family, society and culture to counselor in sexuality; (7) sexual abuse, rape, violence, and harassment; (8) self-protection; (9) practice of sexual counseling; (10) gender identity; (11) exploring the development and experience of counselors in sexuality.
The sexual comfort program for counselors in Taiwan was designed on the second stage, based on the results of Delphi survey on the first stage. In addition, a questionnaire for the program was conducted as well. There were 8 teaching unites in the program, including the diversity of sexuality, sex hierarchy and discrimination in our society, sex language in daily life, my sexuality, dilemma for counselor, the ethics of sexuality counseling, sexual dysfunction, and practice of sexual counseling.
A quasi-experimental design was used to evaluate the effect of the sexual comfort program on the third stage. Sixty-two counselors participated in the study, 31 in the experimental group receiving the sexual comfort program for 26 hours, and 31 in the control group receiving no intervention. A series of ANCOVA and t-tests were conducted to see if there were any significant differences between the two groups.
The major findings of this study were stated as the followings: (1) The experimental group had significantly higher scores on sexual comfort, sexual knowledge, sexual self-awareness, communication skills and willingness to discuss sexuality with clients than the control group; (2) After 6-8 weeks, the effect of the intervention was still significant for the experimental group; (3) The control group had no difference on pre-test and post-test; (4) Most participants in the experimental group were highly satisfied with the program and achieved the teaching goals.
